By agreeing and submitting this application, I affirm that the facts set forth in it are true and complete. I understand that if I am accepted as a volunteer, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal. As a volunteer I may have access to confidential information and documents. I acknowledge that all records are the property of TEAM and will be treated as confidential material. In accordance with this agreement, I waive and release any and all claims for myself, my heirs, executors, and administrators against TEAM Dog Rescue, its Directors, Officers, Volunteers, and any other organization involved, from any and all claims or liability for any personal injuries or property damage of any kind however caused, including any claim or liability arising from the negligence of the organization, its agents, or arising out of and/or in the course of my participation as a volunteer for TEAM Dog Rescue and/or from attendance at any and all TEAM Dog Rescue events. This includes but is not limited to dog bites, scratches, communicable illnesses or pests contracted from any animal. This release and waiver extends to all claims, foreseen or unforeseen, known or unknown. I understand that as a volunteer I am acting as a representative of TEAM Dog Rescue and will therefore conduct myself professionally and responsibly in accordance with TEAM’s Code of Conduct. Furthermore, I commit to protecting all animals under the purview of TEAM Dog Rescue from any harm while in my possession or care.
